In my latest story for Health Affairs, I profile a model of care at a public hospital in Memphis that aims to address the challenges of treating its uninsured population. The hospital, with a team of social workers and nurses, connects uninsured patients with local social services organizations and coordinates their care so they can get help with chronic illnesses outside the hospital.
