The third year of the health law was both the smoothest and the bumpiest so far. More than 20 million people gained health insurance, dropping the uninsured rate below 10 percent – the first time in history that so many Americans have been insured. But premium prices were higher and multiple insurers participating on the exchanges said insuring these new customers was proving too expensive, raising questions about the health law’s survival in coming years.
